I authorize any health plan, physician, health care professional, hospital, clinic, laboratory, pharmacy, pharmacy benefit manager, clearinghouse, medical facility, or other health care provider (“Providers”) to disclose my entire medical record, prescription drug history, and any other health or billing information, including any and all information regarding the diagnosis, treatment or care of any physical or mental condition (“Health Information”) concerning me, to Khatter Financial and its advisors, employees, representatives and insurers.
